Health Care Provider Sessions

Living Healthy Champlain offers training to health care professionals that will help their patients/clients become more empowered to be part of the health care team and that will support their patients’/clients’ own behaviour change (i.e. self-management). 
This is a workshop designed to help clinicians/providers promotes behavior change for the patient 
and adherence to treatment plan, based on their readiness to commit change. Participants receive 
literature, theory skills and techniques.


This program teaches a structured way of interacting with patients/individuals interested in making a concrete action plan for some aspect of their health. Brief Action Planning is based on the spirit of motivational interviewing (compassion, acceptance, partnership & evocation) and its processes (engaging, focusing, evoking & planning).

A session to inform the whole practice about the principles of SMS and to provide evidence for its effectiveness. This session is recommended to organizations that are experiencing barriers in putting training into practice, ranging from lack of provider buy-in to organizational issues.

Health coaching has been defined as “An interactive role undertaken by a peer or professional to support a patient to be an active participant in the self-management of a chronic illness” (Lindner, 2003). 

Living Healthy Champlain offers a series of health coaching sessions to help providers work with their patients in developing confidence and conviction and work through the stages of changes. The sessions can be offered separately in a “lunch & learn” model, limited to one (1) hour or topics of interest can be combined and offered in a “workshop” model, to a maximum of four (4) hours. The health coaching sessions and activities are tailored to the organization’s needs.

Living Healthy Champlain also provides follow-up sessions for trainees to maintain and develop providers’ skills in working with behavior change, and to share best

Workshops are:
  • Free of charge.
  • Easy to register for by phone or by viewing our upcoming Health Care Provider training dates.
  • Recognized as Professional Development and award certificate of completion.
  • Open to all health care providers, including students.
  • Funded by the Ministry of Health and Long-Term Care.
